Anyway, thanks for your response. At least I now know what the problem is.
You could get around it if the alternate keys worked but they don't.
It won't work for me. I hold 'E' and pouch pops up no problem but no key on my keyboard will activate any of the items, top, bottom, left or right, none of them are selectable.
I think it has something to do with changing the arrow keys to movement, being left handed this is how I play. I'm just wondering if the game will let me assign the arrow keys to more than one thing. I will try but I doubt it's going to work.

Switch Right Hand Armament: D
Switch Left Hand Armament: A
Pressing the S key repeatedly will cycle through the quick items you have in your inventory. To use the item press the Use key. I put that key on Page Down.
To use the pouch items you have to hold down the Event Action key. This will bring up the first 4 pouch items on the left side of your screen. Those 4 follow the WASD you selected for the quick items except that pressing one of the WASD keys will use that item. I put the Event Action key on my side mouse button since it must be held down to keep the pouch on the screen. It's default is E.
