For context, her sisters, were "Failed" attempts at creating what i think was meant to be a new "Malenia", with Millicent being the only potential success, and he was actually the one that send off the sisters after her, and will in fact try to convince you to kill her yourself, since the shock from betrayal would be the "Final" step.
In short, hes a piece of garbage.
Regarding her end, Millicent took off the needle since she realized it was turning her into something she wasnt (the whole thing i mentioned above) and she would rather die being herself, which sadly included allowing the rot to consume her since the needle was keeping it at bay, i guess the fight with her sisters was the final push and she couldnt continue anymore.
