The roll button sometimes punches instead of rolling

Since I got the game, all the controls worked fine and there were never any problems. But for some reason, today I played a bit and I've noticed (its hard to not notice) that sometimes the roll button (B, or O since i'm using the PS5 controller) would punch instead of roll. I have my sword equipped, and it punches with the right arm, which is weird. The sword whenever this happens turns gray, as if I can't use it, and it is only fixed when I go to the settings menu and come back, or when take the sword away and bring it back.

This has been happening in the middle of fights and it makes things very frustrating, especially since im at the end-game fighting melinia and the dog.

Does anyone else have this issue? Or is it a problem with my controller (which would be weird since it has worked fine until now and unplugging and plugging the controller doesn't fix the issue - only what i've described above. Also it's a new PS5 controller that i bought just for the game).

Also, any other button works perfectly well, jumping is the A button, Y brings up the quickuse menu, etc. Only rolling goes bad.

when you say punch do you mean the unarmed R1 attack specifically or could you be confusing it for the 2handed or weapon swap animation? also are you holding your sword in your right hand when it happens, like are performing the punch with the sword? also what weapons/wands/seal do you have equipped when it happens

By "punch" i mean the unarmed R1 attack, yes. When i am infront of an enemy and try to roll i notice that i do 3 damage when i press the roll button (and when i attack).
I am not holding the sword, the sword is on my back when this happens. I just have the sword equipped and I see it in the bottom left part of the screen where it shows you equipped items, it just turns gray and i have to do what i've described to reset it.
I use the bloodhound fang 2handed (on the right arm in the menu but holding the sword with both).

I tried replicating it with a ps5 controller and same weapon. I wasn't running into any problems, it seemed to work fine on my end. Maby your 2 handing your bare fist by accident? but dont know why that would cause you to attack with O/B. Sorry I couldn't help my guy, I wish you luck with your issue.

Related but a bit different. I've had some issues lately with the buttons doing the wrong thing too. Sometimes X or A will keep doing rolls if I've pressed them too close to B.

X and R2 have recently started to not respond sometimes. Not sure why.
