Summon to assist = 1 talisman and needle
Summon to attack = different talisman no needle
quitting to titlescreen does the same thing as resting at a site of grace for these kinds of npc interactions. saves you the trip back.
as for the tc : if you fought her you ♥♥♥♥♥♥ up. enjoy your forced frenzy ending. the needle is only given if you help her iirc. otherwise just reset the area to get the needle.
I came back to Gowry midway during the quest and I bought the rot threads incantation he sells, so maybe the game expected me to side against Millicent at the end.
I did not however, trigger the dialogue where Gowry tells you that you must be the one to kill her, so maybe that's why the quest bugged out when I decided not to.
buying the incantation has nothing to do with the quest conclusion as i did that everytime myself. the only choice is if you fight her or help her at the end. the other steps cannot be missed or failed (unless you attack her on purpose yourself) as the quest will simply wait till you fullfill the conditions for her to move on. they also patched killing her after you help her aswell.
the quest didnt bug out. you simply choose to fight her. aka bad ending.
I didn't fight her, do you even read my posts? I even got the rotten winged talisman but the needle didn't come along with it as it usualy does, that's why I'm reporting a bug.
In this case it's definitely a bug. I can confirm that your interactions with Gowry shouldn't affect the outcome of the questline, so besides the usual "check the integrity of the files" advice (which could still work, so try it if you haven't already done it) I'm afraid there's little, if anything, that can be done
Sorry
