the game doesnt end unless you make it so. just to be clear on that. new game plus is not forced.
the cutoff point for most npc questlines is saying yes when melina asks if youre ready to commit a cardinal sin. if you say yes most npc quests will fail unless progressed to this point or finished beforehand.

well I have only killed Margit and Rennala as far as the Main Bosses which opened up the capital for me. I'm level 80. I have not been to the land of the Giants...I don't think.

You'll know it when it happens - you and Melina smoke the universe's largest bowl of Fire Giant Dank and then things get weird

cool..sounds fun.
