I just think I need to position my hand better. Maybe the game is actually playable on mouse and keyboard? That would make it so much easier
I usually just switch targets constantly to keep track of where everyone is, or if I don't know I backup and reposition.

ontop of that change how your position your wrists, since your right hand is doing more you can almost completely sacrifice your left wrist since there is little strain on it, for example i angle my controller about 30 degrees left so my right wrist has almost no bend in it
this config is for xbox elite controllers to eliminate the need to claw as there's 4 extra buttons on the back of it.
Can you not just switch grip when required or is this your only move for 1v3?
When I run backwards like what you are saying my index finger comes down and middle finger takes the position on the bumper.
P1: run/roll
P2: (unassigned)
P3: Y
P4: hold and ABXY becomes d-pad
P3+P4: ABXY becomes your start-menu quick items
Y: 2H the RH weapon
Y: hold for 2H LH weapon
B: crouch

you can mess around with it right now, it's just steam input. with the steam overlay on, shift+tab, select controller top right, or right click on teh game, manage > controller, or do it in big picture mode. 3 ways to get there.
I find that depending on the situation I will have my thumb on the button pad and, index on the thumbstick and middle finger on the bumper... hasn't caused me any discomfort yet.
But when I think about it I used 3 different grips depending on the situation, but when you dodge roll it gives you a window to switch to a more comfortable grip. The elite controller with the P buttons would likely be the best though.
