does it need to be on the same character, in a single play through or can it be done on multiple characters or multiple play throughs?

Pretty sure it's one character only. You can get various achievements on various characters but when achievement is tied to collecting it applies to equipment of one character only.

Almost certain every achievement is a per character basis. Seems to track for where people had to store and then retrieve every relevant item when the legendary achievement/s didn't pop for them.

My bad, I misread. Yeah, you need to collect them all on a single character.
