Carion Study Hall (Realla) what am I missing?

Hi, sorry if I did not spell the names correctly, I just logged off and had them in my mind. I am in the Carion Study Hall, I killed the NPC invader who keeps teleporting away, I went up the rafters to where the rats were and I couldn't find how to get up to the top of the tower.

I guess there is something I am missing but I can't figure it out? Also, the altar at the bottom floor says that "something should go here" or words to that affect, any idea what it is I need?

Complete the Doll Witch's questline you'll recieve the item for it once you're done.

The upper part of that tower is related to a quest MUCH later on

You need to do a questline for someone in the same region. I wont say who because of spoilers.

The quest is in Carian manor, there are multiple ways to trigger it but going there directly and talking to the NPC at one of the towers works too
