Elden Ring is like a D&D Campaign Setting and what that means for an open world/field game
There's all of these adventure point, here and there. With a world that is effectively repopulating with either fresh troops or undying troops. It's an unending playground of factions competing with each other... over someone or something.

Something to consider when you try to judge a world for it's "open world" tag and what you think that needs to be. With all of that in mind, I find Elden Ring to be a pretty damn good world with a variety of beings at varying levels of power that may take you by surprise if you don't have insight or experience from a previous encounter or from another character's advice.

The fun thing is that each player is playing in a world. Each character has their own world. You get to live in a choose your own adventure, represented with graphics and programming, with systems to automatically record and calculate the things that you would otherwise need to write notes to handle.
