It's purely because of poor optimization. The game should be able to run effortlessly at 60fps if utilization of hardware is low, but it doesn't because of inefficiencies in how the game's made. The most common issue has to do with dx12 shader caching, causing framerate drops as more of the game world streams in near region boundaries. If you force the game to run in dx11 (which, unfortunately, must be done offline), the game's overall performance improves at the expense of some other oddities.

We just have to wait for more optimization patches to be released.

My PC is a potato by today's standards, and it's not fully utilizing the CPU, nor the GPU. One would think my rig would be struggling and going 100% to keep up, but no, the CPU rarely goes above 70%, and my GPU averages about 87%. It doesn't go all the way to 60 fps in the overworld in general (there are exceptions), and in dungeons it doesn't sweat to stay at 60 fps. The bottleneck is definitely not in the computing resources. Rendering at a lower resolution than 1920x1080p barely produces any improvement -- going as low as 960x540p gives me only about 4 or 5 fps more, so it's not worth it considering the CPU and GPU could run with a higher load at 1080p.

But the FPS hasn't reached the cap without fully utilizing either CPU or GPU, which indicates, that the bottleneck is elsewhere and likely related to the game's memory optimizations (or the lack thereof).

I can observe the same behavior on my PC's in some areas, especially with particle-effects, like poison clouds or "Night Maiden's Mist".
