My third "arm" idea (Put your other ideas here too)

The third arm could be legs that will make your movement faster and your jump higher. You can also kick with any key (or have the same key as the punch) the kick deals half the damage of the Knuckle Blaster but if you are in the air you can use both of your legs witch will do the same damage was the KB. (Knuckle Blaster) To obtain the legs you can find them in a secret level (only because the time you get the other arms is with 1-4 but that got taken up by the earth mover) the secret level could be in any of the act 3 secret levels

Down sides: if the legs get hit a few times you won't move, to fix this you need to hook shot something and bring it to you and get the blood. After you kill the thing you move but it will be at the same slowness as a Hideous Mass hook. While in this state your health will be halved at 50 HP until you kill 5 things, each kill in this state will make you faster until you are at the yellow leg speed. Oh also you can't jump in this state.
