alternate idea: on ultrakill must die difficulty, the head of a filth will sometimes randomly replace a key, making the level impossible, but also instantly killing you if you hold it.

After passing through the gym, you find him in there, upon seeing you he starts shaking until one super buff arm erupts from his side, and then one from the other. The filth then uses these two arms to support itself whilst it erupts two revolver wielding arms from its chest.
