Even S ranks will still make the hard damage decay fast, you don't even need to maintain ULTRAKILL.
It's meant to be used for mobility, same as before.
You can still use it to grapple to large enemies to get out of bad situations, or can use it to grab smaller ones to get effectively a free kill on them up close. If you keep your style up and use it sparingly this won't get you killed even with the nerf - as long as you don't spam it you should be good. If the health debuff is getting you killed, I think you might be using it too often.
Here are some specific strategies I recommend:
When your style is in a good spot, use it to pull occasional smaller enemies that will die in one shot (the shooting ones mainly) to you and get them with a shotgun or charged pistol headshot or the like. You can also pull enemies off of cliffs and such in this manner.
When fighting the malicious face or such, pull yourself to them when they charge up their "laser" then use an additional dodge or such to get them. I often kill these things by using the homing nailgun shot, then pulling myself to them for a shotgun finisher.
When the giant statues knock you away, pull yourself back to them so you can do more consistent damage.
When you're surrounded by enemies, you can also pull yourself to the status without much of a problem.
All in all it's a risk assessment thing though. I frankly didn't even notice the health debuff until after I had beaten the game despite using the whiplash fairly often in that time (I played after the nerf), but I also didn't use it on actual enemies all that often - only sparingly, where it made sense.
If you need to heal it's best to not rely on the whiplash "except" when your style is good and you're okay with just having partial healing. It's better to instead just get in close and use shotgun attacks and the like, yet it still could help you to set that up by getting you closer (as long as you survive long enough for the debuff to go away).

In Cybergrind for instance, it pretty much becomes my cue to enter into a huge group of enemies and start ♥♥♥♥♥♥♥ ♥♥♥♥ up, since I have the grapple out and won't get too ♥♥♥♥♥♥ over by hard damage.
In general, I tend to not use it if I don't have decent health already, and my main use there is to go for quick OHKOs on Mindflayers and Virtues. The Malicious Face parry is still always available, it's just a lot riskier since you have to time it exactly to not take damage and in the middle of a firefight I've learned to just keep my distance with railcannons and altpiercer charge shots.
You can now also Whiplash coins and shotgun cores. While impractical, you can use the Whiplash to keep a coin in mid-air. You could probably use this for Fistful of Dollaring. Shotgun cores can be thrown into targets or shot in midair way easier while Whiplashed.
You can also quicken rockets you fire by Whiplashing the rocket as you fire it (holding down the Whiplash button) for some fast explosive damage. I believe it travels faster than a Boosted Shotgun projectile.
And when in doubt, you can Whiplash to maintain airtime for a bigger multiplier.
