How to access prime sanctums

I'm sorry if this is a trivial question, but how do you access them? I'm new to this game.

p rank act 1 and go to the staircase part of 3-1 and enter the secret level

You also need to P-Rank all of the Prelude as well (assuming you are trying to get to the first prime sanctum).

all prime sanctums are secret levels accessed by a door in the third layer of their act that can only be opened by p-ranking all of the act's levels (and the prelude, if it's p-1) and beating all previous prime sanctums
