In all seriousness the game has one secret horror game inspired level (and it's really not much). No horror elements beside that.
Some of the ambiance might be a bit spooky but if you're paying attention to your surroundings, you're not doing enough killing.
The second secret level honest to god filled me with existential dread, but it's really worth playing it anyway because it's a surprisingly heartfelt moment from this game.
Basically, light horror elements and some existential dread but they're small moments in between the pounding action and gun play so don't worry about them too much.
