Sounds like your video card isn't "supported" which his why you can't see anything but there's a way around this.

Option 1:
In your Steam library right click Grand Theft Auto IV and click Properties
Click "Set Launch Options"
Copy and Paste this there: -norestrictions -availablevidmem 512 -percentvidmem 100%
Run the game via Steam

Option 2 (no Steam):
Go to the game files for GTA IV and find LaunchGTAIV.exe or something like that.
Make a shortcut and put the shortcut on the desktop.
Right click and go to properties on that shortcut
Go to the shortcut tab
In target, copy and paste this: -norestrictions -availablevidmem 512 -percentvidmem 100%
Run the game by double clicking on the shortcut

You don't need to redownload the game or verify the cache that's BS
