However. asking such a broad question really doesn't help anyone help you and makes it seem that everyone can read your mind.
May I suggest that you give a little more information?
Such as...
Did you try the volume controls in the game menu?
Did you try going to the desktop and making sure the sound is on from your computer itself using the Speaker Icon?
What were you doing on the computer after the last time you played GTAIV that may have caused the sound problem?
Things of this nature give those you are asking a little more information rather than simply asking "Why does my game not have sound?"
Even stating things you have already tried will help narrow the search for an answer as to why, if any, the sound simply stopped working.

I've had that happen to me and it still showed 100% in the system tray of the speaker icon.
I am going to assume you checked the audio Mixer to make sure the Steam Client itself is at a substancial level.
I am using windows 7 so this might differ depending on which O.S. you are using.
Other options I would check is Steam Big Picture mode has a Sound setting, although I am sure it's only use is for the interface sounds itself.
And.
System Sound/Playback from Start>Control Panel>Sound>Playback tab and make sure everything is working correctly that way in their resepected properties tabs.
I really don't think it's the system sound as much as the game.
There could be some sort of conflict between the game and audio driver that isn't showing up.
If I could duplicate the problem or sit-down at your rig I would be able to help a lot more.

Check the integrity of the game first.
Left click on GTAIV from your game list within steam.
Go to properties>Local Files Tab
Click Verify Integrity of game cache.
If there are any files missing it will download just those files.
If you do decide to unintall GTAIV make sure you go into the steam folders and delete anything that is left over before re-installing. You will have to do this manually.
There might be a folder within your User directory i.e. C:\Users\Geist\Documents\Rockstar Games
