Even if you pick "no" that'll not delete the saves you've already uploaded to the Rockstar cloud though, and IIRC you'd get a cloud save conflict dialog every startup. To in fact delete your current saves from the Rockstar cloud you can use "the Steam cloud save trick" of uploading 0-byte large files. I.e.,
1. Close game and launcher (with Rockstar cloud saves still active).
2. Optionally copy or move to safety any SGTA4xx savegame in Documents\Rockstar Games\GTA IV\Profiles\<some rgsc id>\ you want to keep.
3. Replace all SGTA4xx savegames in Documents\Rockstar Games\GTA IV\Profiles\<some rgsc id>\ with 0-byte files: from a CMD-prompt open in that directory, "copy con SGTA401", <enter>, choose to overwrite, <F6>, and again for the other SGTA4xx files in there. Verify with "dir" or the file explorer that indeed they are now 0-byte files.
4. Start the game and on the cloud sync conflict dialogue choose to upload your local files.
5. Exit game and launcher, delete your just created 0-byte SGTA4xx files and the mentioned cloudsavedata.dat file.
6. Optionally copy or move back the in step 2 kept safe savegames.
7. Start game and chose to not use the Rockstar cloud.
Why this stupidly opaque management of savegames by uploading 0-byte files? Ask Rockstar. Or Steam, which does the exact same. Also note the "IIRC" though; if I remember IN-correctly that you then still each time get the cloud sync conflict dialog you may not care about your already uploaded saves and can just keep it at the deletion of cloudsavedata.dat.
- "Profile", with options "Auto sign in", "Hide notifications" and "Other Settings" (a link to Social Club settings on the web, which doesn't have anything relevant)
- "Game Settings", which is other than for the text "No Settings are currently available" fully empty.
Has never been different for me either, although when I googled this back when I in fact played the game a few months ago I found the information saying that it used to be available from there (although I believe in the context of GTA 5 instead). I.e., if you were replying not just from memory, I expect you may want to allow GTA IV to update.

Don't really know what to tell you
Someone else able to confirm or deny the "Cloud Saves" option to be present/absent from the Rockstar overlay (that you get with the <Home> key while in game, "Game Settings" tab) while playing a current GTA IV: The Complete Edition? And on what OS? A current Windows 10?
