That's weird. So for some reason your Steam is linked to the executable, that directly launches the main game. Try this "fix" (well I call it fix, because I don't know what to call it instead):
Go to your game's installation (right click GTA IV, then select properties and go to files, it should take you to where Steam installed game) from there you should see 3 executables, ignore the first two and focus on the last one near the bottom, it should read "Play GTAIV". Launch it, that's the R* Launcher possibly, and it should take you to that screen with the choice. If that executable isn't there, then something's fishy and you've probably downloaded a different version, that doesn't contain the EfLC expansions. I hope that's not the case.
I did try to lo launch the game directly using the executables, but that doesn't make a difference.
I looked all over the menus and couldn't find any new game option, but I'll go over it again more carefully and see what I can find. I'll also check for the skip menu setting.
I am traveling this week, but will try it once I am back this weekend.

Then choose a New Game and there should be 3 options TLAd TBOGT and IV
If not quit andn in steam library right click GTAIV and go to"properties" and game files then verify game cache!
Thanks! This fixed it! For some reason skip menu was enabled by default. Once I disable it I can pick the DLC when I start the game. Also, starting a new game from the "Game" tab in the Pause menu also gives me the option to pick the DLC.
