It said to use DXVK v1.9.4 as it works the best with the game. so I tried it, and now my game works without any microstutters or issues.
Even if you decide to use new version of DXVK or alternative forks, use this config file https://www74.zippyshare.com/v/90lIfaQs/file.html and replace it in your game folder.
Should work out of the box, if it doesn't work, alternatively, try using dxWrapper instead of DXVK.

Also, funny thing, even if I didn't downgraded EFLC, they work far better in comparison to GTA IV (possibly better optimization with the DLCs(?))

Hope this ain't too late to reply and sorry for the bump, I wasn't subscribed to discussion, my bad on that part. As for the issue you were experiencing, it's a bug in the game code, and it is advised not to alt-tab much, however, I'm not sure as I never experienced it. Hope you resolved the issue by now, if not, feel free to update to newer version of DXVK (my last playthrough, I used DXVK version 2.3.1 and alt-tab was working fine.
They fixed a lot of stuff with the newer version, and makes the game run at constant 144FPS for me.
