It happened to me.
loaded the game, went to his mission and crashed. what solved it for me was to do other missions before and when i went back to bernie it worked

Go into task manager, open details and set affinity on GTA IV, then before the mission goes into that loading screen untick all the CPU's except for one then try it, you can turn back on all your CPU's afterwards as it will be laggy otherwise.
