Alt-TAB out of the game. Right-click on your taskbar and select "Task manager" Find "Grand Theft Auto IV (32bit) (4)" and right-click on it. Click Expand then right-click on "GTAIV" and select "Go To Details" Right-click again on GTAIV.exe that should be highlighted and click "Set Affinity" Uncheck <All Processors> and then click only one (CPU 0) is fine but only one is wanted. then go back and "Repeatedly Tapping Space" or "A" on an XBOne Controller will now work to get you up into the Heli...
The affinity trick + a macro to spam space was how I just finished the game. The CPU + 3090FE combo was still too much for the game (even at 8K!), but at least it slowed it down juuuust enough for the macro + me spamming space to do the job.

Come on! That is a hyperbole statement. There are a few issues but the game is still playable on PC via Steam. I am enjoying the game for a few months now.
By the way, another simpler way to fix the issue is to enable vsync and limit the fps to 60. I faced no such issues but for some minor annoying microstuttering.
