Is there any way to block the sexual themes from appearing in GTA IV?

No, but maybe if you find mods to retexture certain characters. Thats all I can think of to remove anything deemed "offensive"

Generally speaking GTA-IV has adult content by default. You will see prostitutes, you will hear strong language. This is simply the way it is. You cannot do anything about the scene at the beginning of the story or any instance where you have to go into a strip club.

If you are allowing a minor to play this, please be advised that the game is rated 18. Be advised that Grand Theft Auto V is also full of adult themed material and will definitely offend. If easily offended, I advise not playing these games.
