Report a translation problem
Game folder > Right click on GTAIV.exe and enter its properties screen.
Under the compatibility tab, tick Disable fullscreen optimizations and click OK.
It worked for me.
part of is a conflict between intel hd and Geforce cards...read full post ..too much to cover here.
Open the folder where GtaIV.exe is placed on your computer. this can be found with steam by rightclicking the game in your libary and clicking on properties. find "open file location"and left click it. Find the file GTAIV.exe and right click it to get into properties. (somtimes you have to load this twice to get to the compaibility tab) once you are here look for the disable fullscreen optimizations click box. have it checked and apply the settings.
Run the game in windows vista Service Pack 2.

*If you are useing a nevida CPU*
-going into your nevida control panal and manage your 3D settings.
Right click the back of your desk top and left click on nevida control panel
find the program settings tab. you should see a drag down bar where you can find GTA4 and slect a preferred processer.
Use your High-peformance NVIDIA Processor and Apply the settings.
**make sure you have updated Drivers with your GeForce Experience program, u can find this with your search tool in the bottem left tab on your screen **

Right click the game in your steam library. Left click on properties. then left click on set launch options.
