If it was multiplayer i believe afking will kick you otherwise it may be a scroll combo you had that dealt damage. Not sure what it could be though.

Pretty sure every self-damage scroll specifies it can't kill you. And most of them require you to actually press a button.
The only ones that passively damage you are the "being far from a teammate" one and airbag, which leaves you at 1 and only if you have shield/armor.
The only ones that actively damage you are bloody ammo when shooting with no ammo, overdraft when throwing secondary with no charges, and a curse that has a chance to damage you when shooting. But all of these can't bring you below 1 HP.
The only one I know of that can kill you is Fire enthusiast. But that only works if you actually inflict burning on an enemy.

Most likely this one. I avoid it because my partner and I love fine-tuning builds and we stay waaaay too long in one place lol
