It's from stage 1. It's supposed to be a hint on how to break the floor, which you do by just jumping place over and over (I want to say 50-60 times?).

Wow, I never saw that. There isn't enough dead space around the QR code to scan from a distance. I trimmed it down in paint and it scans OK.
