Swell is the basis of playing mage.
I don't see how Swell and it's upgrades are any more powerful than just having shield, freeze, or stun cards, it's one of the only ways to get health back, and its very niche as you need to make sure your incoming damage isn't lethal and that you will heal more back than you take in damage for it to be worth getting hit in the first place. Swell already has a soft cap on it in the form of how small it heals, 5 hp from your starting 30, per Swell card, is barely anything when most mobs towards mid-game start hitting easily for more than that because of growth effects or just already having higher base damage, or larger amount of mobs spawning in general which hit you for a higher total.
If the concern is more with how it synergizes with things like Thumb Lancet, keep in mind that many things in this game synergize like this with more powerful effects.
There are multiple artifacts that can heal you. Ancor can heal you up to full health when you are at 33% (or bit less the lower you get), which singlehandedly can allow you to abuse thumb. There are relics that heal you when you exhausts cards. Which heals when others are healed. Etc.
Swell is pretty hard to actually get value out of it when the floor gets higher. It may be stronger in non-endless, yes, but even then getting it to a point where you get 2 or less damage so that you actually heal is not as easy
