Knife-only run

For the purposes of the Knives Out achievement, does holding a gun without firing it count as using one?

you can "use" guns, just don't use them on an enemy same goes for pipe bombs, mines, hand grenades, flash grenades.
you can still use them to solve puzzles

You can hold guns, and you can even fire as long as you're not shooting enemies (you can also shoot Goats of Warding). Just make sure you don't shoot enemies other than Heisenberg when you're not in the tank, or Miranda's projectiles iirc.

You can check your challenge progress. If you have any doubts pause the game, go to records, scroll down and you'll see if you did something that nullified the challenge.

So how should he shoot with the gun if he's not holding it?

There are instances in the game where you're forced to shoot.
-Bells puzzle requires shooting
-3rd Sister requires blowing the wall with an explosive (just make sure she's far enough to not take any damage from the blast radius)
-Heisenberg must be shot while you're in the tank and can't use the knife
-Chris needs the Target Locator to destroy the megamycete, enemies dying from the blast radius is fine, as long as the player targetted the megamycete and not the enemies.
-Miranda's fire balls, or whatever they are, can be shot. They don't count as enemies.

So yeah, it's basically "knife only" except for things you absolutely cannot do with just the knife.
