The old lady at the beginning of the game, the one that says "In life and in death, we give glory", was that Mother Miranda? We see Miranda turn into her at the end, so was it her, or was it just her mimicry?

Yes, that's her. Well the old lady could really exist and Miranda may had been transforming into her, but there's no such indication.

The real old lady was the friends we earned along the way.

It was her clearly. I mean the entire village got slaughtered, even the able bodied people who had weapons. In that respect I doubt an old woman hobbling on a walking stick would have outlasted and outmaneuvered the lycans.
