very hard is not much harder than hard so you might as well do that. I found i need to run from Boss BTs (when you get dragged by a normal BT) but otherwise, normal BTs and humans are fine to deal with
Also on very hard timefall comsumes your equipment very fast, so carry only what's needed and be ready to recraft a lot of things. Cargo is not bad since before getting damage the container must be gone.
Since there's no penalty in switching difficulties if someone doesn't want to bother enemy bullet sponges and equipment turning to dust superfast it's good to leave LLL deliveries for when you have everything connected with a zipline.
About enemy bullet sponges. The only enemies that has a lot of health on very hard are the bosses, which personally I think is fine because they're bosses. Normal BTs and humans goes down fast. I cleared a MULE camp consisting of ~16 terrorists with a single level 2 assault rifle (Lethal), no extra ammo packs. And Bola-guns and backstabbing with the strand still one-shots humans. Normal BTs take 1-2 blood grenades or 1 custom blood grenade.
According to the game you also gain extra loot from enemies on very hard. I'm not sure exactly how it works because it doesn't happen if you shoot them (lethal), but it does seem to always happen if you take them down with the strand or bola-gun.
