to the southwest you have the orc nation
to the south you have the vampire nation
to the southeast you have numeria, that can hold the demons back as long their tech won't get stolen
and to the east you have mendev, the crusader nation
honestly beside being the closest target to the worldwound, mendev also looks like the weakest from all of them
The game focuses on Mendalev because that's where you are as the main character. Maybe there are other heroes having heroic battles in their own nations to stop the demons crushing things that are important to them.
Presently The Bugboi seems to be quite a bit desperate about Iomedae not noticing him whatsoever. So there's that. The Goatboi is probably tagging along like "I knew this was a bad idea from the start, but... aargh, f@$#, I'll show 'em, I'll show 'em all!"
Deskari's obsession with Iomedae is due to his utterly humiliating defeat at the hands of Aroden. The god who beat him to within an inch of his life and threw him down a hole. The god who Iomedae previously worshiped, before he was supposedly killed and she ended up replacing him.
Unlike many other demons and demon lords, Deskari was never born mortal. He's actually the offspring of Pazuzu and some other deific level being. This gives him a massive superiority complex over mortals, and the idea that Iomedae who was previously born mortal is a worthy replacement for his most hated enemy is horrendously offensive to him.
But, arguably, it's more funny that he's just desperate and is showing it in the only way he knows how. Or I may have been reading too much Pathfinder memeposts on Reddit. Also possible.
The Mammoth Lords are just bands of tribal clans but they're incredibly skilled warriors and the local wildlife is also pretty damn strong. So any demons attacking them would be taking a huge risk for the gain of... a few slaves? Fur pelts and crude weapons? The bragging rights to say they did it? Pretty terrible deal,.
Belkzen is orc territory. They have more things of value to take compared to the tribal Mammoth Lords, but orcs are a warlike society where everyone can fight back. Not exactly easy to take down. Plus some of the orc tribes worship other demon lords, so Deskari and Baphomet have reason to keep their forces away. They have enough enemies with the crusade and don't need to worry about another Lord being pissed at them because they killed someone else's worshippers.
Ustalav is protected similarly to Mendev, plus even if the demons did get through they'd have all kinds of other monsters and undead to tangle with.
and beside vampires ustalav also has a softspot for other undead beings
they are also stuck between orcs and barbarians, because of that they have history and experience with fighting savage opponents on a regular basis
they also have a geographical advantage with their capital being really far away, if the worldwound expands they lose it after 6 months while orc nation and numeria already lose it around 3 months
from all neigbhouring countries they are pretty much on second place in holding against the worldwound only surpassed by the mammoth lords
Aside from not being able to withstand the sun, Vampires also possess a limited form of invincibility which is similar to that of Liches. As long as a vampire can return to its coffin (or equivalent resting place), its body can regenerate back to full health.
According to the Archives of Nethys, the method for permanently destroying a vampire is to stake its heart (which paralyzes the body), cut off their head and then douse it with holy water.
You can also expose them to direct sunlight, but this doesn't instantly kill them, and more than likely the vampire will try to fight back during the process.

More over, vampires are immortal, meaning they can have centuries, or even millennia worth of combat experience. Plus the ability to summon and control various minions to fight alongside them.
And while demons may not be easy to dominate, the same can not be said for the weak minded cultists who follow them.
So vampires are only part of Ustalav. They also have weird necromantic/steam punky tech and not so hidden Urgathoa worship. There also is the tendency for Hastur and some of the other Great Old Ones to occasionally influence the area. Plus I'm sure some demon princes aren't too keen on waking up Tar-Baphon
So everyone is kind of up playing the vampires in Ustalav, yeah they are there and yeah they are a problem but it's only a small part of Ustalav. Hastur and other great old ones are worshipped in secret cults there. There are two major secret societies there as well as pathfinder likes to do. There is the Esoteric Order of the Palatine Eye which are occult masters and there is their enemy cult The Whispering Way which are loyal to Tar-Baphon, the whispering tyrant who is an extremely powerful lich that was originally killed by Aroden himself. So there are probably demons not too keen on going to far south into Ustalav just for risk of waking Tar-Baphon.
Additionally around this time Stasian Coils have likely started showing up in Ustalav which would let them make undead constructs pretty easiliy
