It's also used if you're struck while casting to successfully cast, or when you're taking continuous damage.
Alternatively, if you do not want something to distract you while casting in combat, you do a concentration check with a DC of 15+2*spell level.
The roll is d20+caster level (important, can be changed by negative levels)+attribute of the spellcasting class.
So if you want to defensively cast magic missile it would be a DC 17. A Level 1 Wizard with Int 18 would roll d20+1+4 to beat 17. So a 12 or better.
Taking damage while casting gives you a DC of 10+damage+spell level. Same wizard, same spell while taking 5 damage is DC 10+5+1 = 16.
There are many more DCs for various distractions, but I do not know if they coded them into the game and how.
If you fail a check, the spell is gone.
