Once saved, go to the Saved Outfits and then select ones you want to be available via your Horse Saddle.
In Story Mode you can only save 4x Custom Outfits.
A horse saddle will hold 5x maximum if you apply Upgraded Saddlebags first.
But not all trainers will have the same options. Look up Lenny's Trainer for RDR2 on Nexus Mods as most mods call for you to use that as a requirement anyways
Same sort of glitch as when equipped weapons suddenly are not being carried on your body anymore. Quite irritating when you didn't realise that, and suddenly enemy or huntable or aggressive animal shows up.
Stiil, if you realise it can/will happen, game stays very playable.
(Sorry, missed your Modquestion, but I have no idea how Mods work with custom outfits)
Make sure you use Upgraded Saddle Bags so you have 5 horse slots of outfits. But only load it up with 4 outfits. This leave the 5th slot open as it needs to be because the game needs that slot for Custom Outfit when you put a face mask on, remove gloves or hat.
