Why can't I rob the clinic in Valentine?

I'm at Chapter 4 right now and I just fast traveled back to Valentine because I never got around to robbing the clinic's side business. When I walk in I can't pull my gun out and there is no option to rob the doctor when I look at him. There's an option to press and hold LMB to draw your gun, but Author doesn't mention the side business or anything, the doctor just runs outside.

Already did that, twice actually

After you did that you should get an option to rob the backroom when talking to the doctor. If that doesn't happen, something is bugged I guess.

Maybe try to sleep and return to the clinic a few days later to see if that resets whatever has gone wrong?

I'm trying to remember. Didn't it take somebody to tell you about the illicit side business first? Did that happen?
