The gang has always been doing awful things, robbing, stealing, killing, using people etc. since the beginning, even Arthur says "ive killed ALOT of people in my life" numerous times. Dutch didnt change, I think Arthur's different look on life after finding out he had TB opened his eyes to just how evil Dutch actually was.

Nah he shows signs before that. He has Narcissistic Personality Disorder; tending to use "I" over "we".
Like at the start he tells Charles "I- we need you strong". Then after fishing on the boat he says "I think i-- I mean we, are gonna be okay". Then his constant mention of this unclear plan and goals of grandeur. John even mentioned Dutch killing some innocent woman in Blackwater before the game even started.

Dutch was always this way. He was never a good guy. He seen himself above everyone else.

I've seen many theroy videos on YT saying that Dutch may not have been a good man, but after that trolly accident he just got way worse. The gang was doomed from the very start of the story because of the way Dutch thinks, it just worsened. And Arthur starting to see things differently only adds to his changing perception of Dutch, seeing things as wrong that he previously didn't. It's a culmination of both I'd say.
