If you are exploring the whole map, you'll eventually find it and the game will tell you when you have it. You can look in your Tasks list for such things (L key)

Don't forget to use Eagle Eye when on your travels, you may well uncover something you didn't even know existed.

Some of these things are only triggered at a certain time of day, talk to Dutch at the right time and you'll get the "quest" for his pipe.
Some may also be chapter specific but I'm not sure.

Enjoy the game, the world and the experience mate, it's not called a masterpiece for nothing.
They will come to you at some point during actual story mission progression. While I do not recommend players to rush the story. After a story mission it's good to go back to main gang camp and see if any unique side quests are available. But also sometimes those don't come available right away sometimes it depends how the gang members are moving about, conversing with one another, time of day can all play a role in these side quests becoming available to you.
Usually after a story mission, I'll go to places such as Valentine to take a bath, get a hair cut, buy some ammo, play poker. I will sometimes go towards the frozen lake just south east of Colter to hunt Elf, Wolves, have a tussle with a Grizzly Bear (as you need to take down 18 Grizzly with Knife for achievement; Grin And Bear It). Or maybe go to Strawberry as a quicker means to get to the northern side of Big Valley as there are Grizzly Bears there as well.
Not to mention taking time to stock up on herbs, cook seasoned food from Succulent Fish Meat or Big Game. I rarely visit a Butcher as you can usually sell just about all your animal related finds to a Trapper, who you will need to off load various items to anyways if you want to eventually be able to unlock and buy all the unique clothing items has has available over time.
Before you sell items to Trapper, be sure to check with Pearson at your gang camp, see what's needed for the upgraded Satchels so you can eventually do all of those to unlock and make the Legendary Satchel so you can carry 99 of each item.
Then there is the Fence to visit who has dynamite and other items and can craft the unique talismans from legendary items and such.
just an fyi tho, none of these are required for 100%, and you only have to do 5 of them to get the achievement.
