Many games have optimizations tied to their in-game Vsync implementation, you should always leave Vsync on as it's the only way to guarantee a tear free and smooth visual. VRR technology like Gsync and FreeSync do not eliminate screen tearing alone, you still need Vsync turned on for a completely tear free experience. Gsync and FreeSync users should also cap to -3 or -6 below their refreshrate to keep Gsync engaged.
Anyone who tells you to turn Vsync off, has no damn clue what they are talking about.
Use a program like RivaTuner Statistics Server to cap your framerate to your lowest regular frame drop for the most consistent visuals.
If you get 120 fps in one area, but 60 in a more demanding area, lock your game to 60 or you will get animation stuttering from frame fluctuations.

Yes in 2023 if you have a 240hz monitor leave vsync OFF for competitive games. Ex Pro Quake player. For RD2 you can leave it on. Screen tearing is not an issue in most game with a 240hz monitor and mouse lag is a real issue when spinning 360 for a fast shot. You do want to decouple the mouse from the equation.
This is all HIGHLY subjective. No system is the same, even with identical hardware (which by the way is also why making games that run well for PC is much harder than consoles)
Realistically you should do what feels right for your game. For me doing but use in-game Vsync makes the game feel choppy and jittery. Input lag isn't a big deal because I play with a controller.
