It showed to me when I was in Epilogue as John, but you still can get this as Arthur

if it's been beyond 3 days, does that mean I can't find the vampire in that spot anymore?

Every time I showed up before or just before 1am he never spawned in. For me it seemed more like 1am-2am.

You will know when he does spawn because a Black X will show on your Mini-Map Radar to indicate the body that the Vampire is standing over, suking on his neck just before you walk into that alley-way. If you don't see that X on your radar, don't walk into that alley way or you'll basically force him to not spawn-in.
