Cores get depleted as you or your horse get hungry
You can only decrease the drain rate for horse cores using a better saddle and trinkets
Cores must be replenished by food, tonic, herbs, or sleeping.
To help keep Horse Cores up, spend more time while riding him/her Calming them and Brushing them. It even tells you that if you continue to allow Horse to be dirty, the health core will lower faster over time.
Overall it's not a big deal because you can literally play the game using the same horse all day long and the cores will never go down into the red. Unless you do something foolish and have the horse go through a near-death experience or get badly injured.
What they should have done also was have various types of horse shoes as well. They had this in Kingdom Come Deliverance
