No, you only risk to become overweight due to all those juicy gator steaks, not underweight. Why would you go without eating? Food is all around you.
You need to eat regularly to maintain average weight, that’s it. You won’t lose weight, ever, if you eat your breakfast, lunch and dinner and maybe supper if you had a lot of riding and fighting that day. And if you lose weight for some reason, it is easy to regain it within one-two days. And I mean, REALLY easy.
I've only ever gone underweight after not eating for a couple of days, though I don't check every day so it's hard for me to notice either way.
My advice: hunt anything, and everything, always have your food stores maxed out for both cooked and uncooked meats, simple cook is enough. Use herbs only for the best foods so you don't need to farm them as much.
Even 1 star game is fine, just carve the meat. This is especially true for small game which you just keep in your pocket and don't need to stow the pelt; since they stay in your inventory even after death unlike medium or larger pelts.
Added to this is the time mechanic.
For example you can spend ten or twenty minutes of real life time perusing say the Trappers wares but in game time the clock has moved on a day or more and now you're thin and so is your horse.
