One example is what you just mentioned: trains. The moment you try to take over a train the clock is ticking and you have virtually no time to do anything, the law will be on you extremely fast even if there are no witnesses/no one left alive/ no place for anyone to run to. You can succeed at robbing a train reliably only by cheesing the game (park the train in a tunnel), otherwise the lawmen almost immediately know what's going on and practically teleport to you in a minute (the tunnel only keeps them from getting to you).
Another example is the back room in the Valentine doctor's office. The back door is not possible to open even from the inside, killing everyone and looting everything as fast as you can still won't be fast enough to escape before the law shows up, and your take from that operation is not likely to break even with the bounty placed on you when you try to escape, unless you save scum your way out. Just not worth the hassle.
There are more examples, but Rockstar overtuned quite a few things in an effort to prevent the player from just easily being able to roll around in tons of money. Which is stupid for two reasons 1) it's a game about being an outlaw and they make it infeasible or not worth it to be one in a number of places and 2) you make tons of money just doing story missions.
Edit: also, yes, the mask thing is another bit of nonsense that every new player (me included) has to find out the hard way. The mask only helps keep your identity hidden from normal civs, which prevents them from reporting your identity to the law. However, the law themselves see through everything; the moment they show up, you will get a bounty on your head if they see you, mask or no mask, disguise or not.
