I have LML + the Stream folder set up and working with various mods, so I don't mind installing the mod you're trying to use, see if it works, and then get back to you with the steps.
It's recommended you install Lenny's Simple Trainer along with Lenny's Mod Loader just incase (Since some mods use MP content that isn't loaded normally in SP which Lenny's Simple Trainer Fixes)
Anyway, make sure you extracted the "ModLoader" folder NOT the "ModManager" folder to your game's directory.
And I mean the contents of the folder itself, not just putting the entire folder itself into your directory, that doesn't work.
First LML needs to be installed, as you already know. Small disclaimer, I'm personally using version 6 because it's worked fine with all my mods so far and so I haven't had a reason to update it. I see several versions have released since, but I haven't used any of those. Feel free to try newer versions yourself. https://www.rdr2mods.com/downloads/rdr2/tools/76-lennys-mod-loader-rdr/
First off, after unzipping the LML mod, you need to put everything located in the ModLoader folder into the game's directory. Everything except for the ''_PLACE ALL THIS IN THE GAME ROOT'', this can be ignored. After that's done you go into the ''lml'' folder and create a folder called ''Stream''.
After that you need to unzip the poncho mod, take all 4 files containing the words boar tusk, and place those into that Stream folder you just created.
Afterwards you go back to the contents of the unzipped LML folder, and place ''ModManager'' in your Documents, or on your Desktop, wherever you want it. Once it's located somewhere you open the folder, click on ''ModManager.UI'', choose an interface color and optionally enter your game directory path. Mine always finds it automatically, but I do have the game installed on my main drive, should that matter. If your game isn't on your C drive and this is giving you trouble then I recommend moving the ModManager folder to whichever drive the game is also installed on.
Lastly, you'll need the ''version.dll'' file. I got mine from Lenny's Simple Trainer, like Swiggity also suggests. Download: https://www.rdr2mods.com/downloads/rdr2/scripts/7-lennys-simple-trainer/
Out of all these files only the version.dll is needed, but if you're interested in the trainer as well you can transfer all of them. The version.dll + the other files go into the game's directory.
This should be all. Feel free to let me know if there's an issue.
Just wanna say thanks to y’all for helping me troubleshoot. It works fine now. It clips on the right shoulder unless he’s super underweight like people said, but it doesn’t bother me much.
It's funny you bring up this specific mod 'cause I actually used it to create an outfit with the poncho from Nexus earlier today. Any outfit created with it can be saved and quickly accessed in its menu afterwards, so you can have, I dunno, 30 of them if you want instead of the regular 4-5 the vanilla game lets you save and carry.
If you care about your trusty steed there's also the Pimp My Horse mod, which lets you use MP equipment in SP, and then save custom creations like with the Outfit Changer mod.
There are other mods that do all of the above, albeit in different ways, but I personally prefer these two for my own play style.

It doesn't change any stats or anything else really in regards to gameplay.
Now if you were installing something like Ped Damage Overhaul that would be another story.
