Anyone else find voice chat to be very low volume and hard to hear?
I have everything set in the options to max, and all my Win10 options to max also, but I can rarely hear anyone in game when they speak. If the name didn't show up on the right when they spoke, I might even think it was an NPC far away saying something lightly.

Anyone else experience this, or figure out how to make it better/louder?

It sounds like you have enabled 5.1 audio over Stereo. Then this kind of thing would happen.

I'm new to Win10, so I might be missing a setting somewhere, but the only options I see for my headphones are under "Spatial Options", which gives me none or Windows Sonic. I have this set to none. Do not see a 5.1 option, if that is the spot it would be listed.

I did however turn on Loudness Equalization. I remember in Skyrim, if that was off, I couldn't hear NPC's when I was looking directly at them. I'll see if this helps later...

Open "Sound settings"
Press "Sound Control Panel" up in the right corner of that window
Choose "Playback"
Click on your audio playback device
Press "Configure"
Choose "Stereo"

Configure is greyed out, but been through all those settings otherwise.

Just a cheap pair of Logitech headphones (well $60 got for $20).

Haven't had anyone talk in the past couple of hours, so can't tell if loudness equalization fixed it or not.
