With NPCs, it works every time, and I have the timing figured out now.
So it's E, then press one of your movement keys and keep it pressed, then hit R, as quickly as possible.
With no obstacles around, it's just a big shove you give the guy, perfect for shooting him with wild hipfire.
But in the saloon during the mission I actually threw somebody out of the window again.
It seems that it depends on what is close to you, sometimes I smash their heads against a wall this way.
Then later I tried with a random human player who probably couldn't understand a word I said.
When I tried to do the throw he would punch me before I had a chance to make it work.
There is still so much unclear about the melee system, I kinda suspect that it is sort of like rock, paper, scissors, with your E, R, and F keys.
No, not like that.
During hand to hand combat.
Melee.
You grab the person and then swing them around in the direction you choose with your movement keys.
And if that is pulled off on a roof or a bridge, you send the guy flying.
In saloons you can throw people out of the window, if you are positioned right.
