Is my wife secretly infertile? Am I?
I am 37 and my health is fine, my wife is 31 and healthy, we are soulmates, we have been married for 10+ years, i have laid with her multiple times through events and certainly even more times through behind the scenes sex (from being soulmates and married), still no children

Probably not. Barren should be a visible trait. Seems more likely that you've just been unlucky. I assume it didn't say 'chance of children: none' when you got married?
I didn't choose the marriage myself, when he got married he was my grandson and the heir to a title of his liege (my son) and as such I could only arrange a marriage with someone in my court, which is all incest lmao.

I did just go back and load a save i happened to have before i got married to check, chance of children was medium possibly due to the 0.9% chance of inbreeding they had so i suppose it really just is bad luck. I'll keep trying. Worst comes to worst I can bump up to absolute authority before I die and pick an heir.

The inbreeding chance doesn't affect fertility rate I don't think, and 0.9%'s pretty low anyway.

It's up to you at the end of the day, but I wouldn't land my heir as a rule. You end up with things like this happening where you're not getting to choose marriages, they're getting rivals, accumulating stress, having affairs, people are getting hooks on them etc. and more often than not it's kinda unhelpful. The potential negatives of doing it typically (significantly) outweigh the positives. There are a few government types (the admin ones) where it's kinda inevitable, so you might as well, but otherwise, I'd avoid it.
It's entirely possible one of them has low fertility. Seduce someone to see if it's your character.
I've never experienced any "secret infertility" issues, so it's probably just you being unlucky, like mentioned.

Gifting your wife +fertility items - or wearing them yourself - will boost the pregnancy chance.
Pressed asters can be found during hunts and make for excellent gifts for your loved one. Rings and other trinkets with +fertility can be won in tournaments.

PS. Did you remember to soak your genitals in cat's milk and say Ave Maria backwards, before laying with your spouse?
I'd say either unlucky or a bug.
i solved all these weird fertility mechanic issues by installing carnalitas mod that let us literally have a "do it" right click option on a character (because otherwise my beautiful young soulmate couples seems to really have no clue how sex works)
NPCs will equip gear that you give them if they have an open place for it, so you can often increase the fertility of your spouse by ~50% by shipping them pressed flowers and maybe a good nasty book.
If you have debug info enabled, you can see the character's actual fertility % in tooltips. They're probably both low.
Murder your wife (ingame) then marry a new young and fresh one.

Pfft, doctor shmoctor. Everyone knows a few thousand hours of CK3 is basically the same as a decade of medical training.
