I don't remember exactly but doesn't she go to the crypt after the event, if you choose Tristan instead of her?

After you get Tristan in that one mission a few levels/missions down the line a mission will pop up with him requesting to get her corpse, once he completes the mission and comes back he again requests of you if he can lay her to rest in the crypt in Camelot, by which then you "have" her, you will need the Resurrection book if you want her in the flesh with you

does it happen in act 3? i'm now on last mission of act 2 though.

she is lvl 10 if i remember so prob unlocks in act 2. you must have done the 'event' that allowed Tristan to bring her body back home - so you needed to have Tristan in the round table at least until then (around lvl 9 - 10ish missions). Then she will appear in Camelot's crypt (like any other dead knight) and can be ressed with a spell/book.

i have tristan quite early. pretty sure before act 2 i already have tristan in the round table. i also recall seeing an event about isolde from tristan. but she does not appear in the crypt. i have the tome of resurrection though.

so isolde is missable? really not sure how or the correct sequence to get isolde. or is this a bug?

ps. i'm in chapter 3 now by the way and already level 12 now

Can you provide a screenshot of the crypt in your campaign?

The slot with the "RIP" is where Isolde is.

Just mouse over it (a little too subtle).

i had her right at the start of act 2 (or end of act 1).
Started with going down to the shipwreck, however you meet her lover, he free's her and then during act 2 moves her corpse to the cathedral crypt (so far she had not been usable), she is dead, but i was told you can find resurrection scrolls. However my list is full, i have 1 in the 4 slot backup BUT if i want to free a slot i have to permanently remove an ally to bring a new one in (this is also not even resurrectable, its gone from your game permantly on that playthough), hmm nah i dont like that.

You should at least be able to move one out of there, but not lose them forever.
