i feel like they originally intended to make her look good. but then at the end thought : " oh no, we gonna get in trouble with the press if we create a hot MC. so lets just uglify her and make her look worse than an average unimportant side NPC in the witcher 3 "

because you can tell ALOT of design went into her body, animation, movement etc. but the face for some reason got no love at all. i dont think thats coincedence. i think they changed her design in the later stages of the game development. but i could be wrong. but thats the only way that makes sense. why else spend so much time on everything EXCEPT the face?
