This can happen when targets are far away or around complicated geometry. When you use VATS, your game camera zooms in a bit. This can suddenly cause whatever you were targeting to become obscured by terrain or things like trees, which will then end up breaking VATS. You'll wanna try getting a bit closer, or move laterally around the perimeter to get a cleaner shot.
And sometimes it's just a bit janky, despite a clear shot... but usually the same methods will resolve it for ya.
Do you have a VATS weapon?
I use vats a lot for score and it has never been an issue.

Things like bushes, grass and other objects can cause vats to not work correctly as it messes with the line of sight to the target. Also certain enemies due to their own body shape will block the targeting depending what body part you are aiming for. ie targeting a head that is blocked by the body as you are aiming from behind them.
Vats should just work. It doesn't require any perks at all to simply function but of course they will make it better.
Try different weapons. Try it in different locations and with different enemies. Move around so you are targeting enemies from different directions and engaging from closer to them. See how you go after trying these things.
VATS works fine. Mostly. Though it works better with some weapons (most automatic ballistic rifles/pistols) than others (like most heavy weapons). It also works better with third person melee than it does first person melee, do to first person focusing on the waist of the enemy making it hard to see anything.
It works perfectly fine, believe me.
I can even switch it on an off to target and kill different enemies as fast as with the Gun-Fu perk.
Works as intended, my Bloodied Commando build depends on VATS and it works perfectly for me, no issues.
