Can't Start "Into the Mystery"
I don't have the veil or the holotape on me. I've been to 14 of the spawn points, and it's always just the tattered dress. Are there prerequisites to be able to do this quest?

Well it's a random encounter so maybe go to one dead young woman location and keep logging off and on again because the prerequisite to start the mission is to "Find a body with a Worn Veil and Damaged Holotape"
You can give this a try, and it may or may not help, find a spawn for her, loot her corpse, then close the loot window down, then wait a minute or two, then try again, that got the holo tape and veil to appear for me when doing it a while back.
If I recall it's the corpse at Lewisburg that you get the holotape from, It's next to a vending machine.

If the young women corpses are empty it means you've already looted them (a downside/overlooked interaction to area looting).

Check your inventory and/or stash for the holotape. If it isn't there... chances are you threw it away? But being a quest item that shouldn't be possible.

The ones on the wiki are the fixed spawn points; those are the ones I checked.

Every time I went to a new corpse, I checked my inventory; the veil isn't there.

How low of a percentage chance is the random encounter? I went to FOURTEEN different corpses.

How long ago did you do this? My first character did the quests, but that was 3+ years ago.

Out of interest I went to a known spawn point.
Summerville, a place that my character hasn't been to before so upon arrival discovered it.
A small quest showed on the map (Bring the music back by turning on three jukeboxes).
I did that while eliminating the resident blood Eagles then went and inspected the corpse of the young woman in the parking lot on the East side of town.
The corpse only contained a tattered dress.
A moment later another quest notification popped up and the corpse now contained the quest items as well as the dress.

No idea why this didn't happen with the fourteen corpses I found yesterday, but I went back to Summersville today, and the quest popped up moments after finding the corpse. I've been trying off and on for a week to get this quest to activate; I don't know why this time was any different. Thanks to all for the help, although I can't figure out what the problem was; I didn't do anything differently today. At least I know it still works...

Last year I think, pretty early from what I remember.
