I catch an illness and it doesn't count for me in the challenge.
I got Mud Lung and he doesn't tell me about it in the challenge of contracting 22 diseases.
Do you know why this happens?

u have to catch ALL 22 at some point or another. Check the challenge (expand it) to see which ones you have NOT caught(likely oopsie or the other one, both can be caught with baby mirelurks, just go to the area and kill all but the eggs, unequip all armor and then go near the eggs and let them chew on your legs for a while till u catch one, switch servers and repeat to catch the other your missing)

Nope, checked and I'm only missing Sludge Lung. Had that a hundred times and never counts. Just ran up to ask heap and hung around the mine and got it again-No Go At that Station.

I saw a YouTube video on how to get the Sludge Lung to register. I looked but couldn't find the exact video again. iirc, you have to server hop after you see the Sludge Lung message and drink dirty water until you contract parasites, then Sludge Lung should register. I have not done this so I do not know if it works but I have seen other comments and doing something similar to get it to register. I might give it shot today and update.

I went to Belching Betty and contracted Sludge Lung but it still did not register as expected. I hopped to another server and drank water off my hand pump at my base and got Dysentery. I hopped server again and upon logging in I saw the message about Sludge Lung and Dysentery. Checked Challenges and now I have the Sludge Lung challenge done. Just 3 more to go now.

That happened to me too. Sludge Lung wouldn't register as an achievement even though the game said I was suffering from it.
I tried a few things like making myself vulnerable and getting it again, then walking over poisonous spikes.
Didn't work.
Maybe there is a time component to catching another disease while you are infected because after that I tried visiting Whitespring where the diseased ghouls are while suffering from Sludge Lung and let them beat me up. One of them gave me another disease which caused the Sludge Lung to register.
